Consultation notes, clinical basis, and a cleaner workspace

- Consultation notes. After each Guided Consultation, Steady generates a summary you can review and edit. Save it to the client's profile and it becomes context for future sessions — so you don't have to re-explain the clinical picture every time.
- Clinical basis on every response. Each response in a Guided Consultation now includes a "Basis" button showing which frameworks informed the response, what client information was referenced, and what gaps remain. Fixed an issue where this wasn't appearing reliably.
- Collapsible client context. The client presenting concern at the top of a Guided Consultation is now collapsible. It starts expanded and tucks away once the conversation begins, so it's there when you need it and out of the way when you don't.
- Cleaner consultation summaries. Summaries no longer include stray formatting artifacts or raw system data. They read like plain-text clinical notes.
- Streamlined signup. Removed the phone number field from registration. We'll add it back when SMS verification is ready.

Library uploads, profile cleanup, and infrastructure

Security
- Library uploads are working again. Fixed an issue where some PDFs were failing to process after upload. Your documents will now extract reliably and become viewable in your library.
- Cleaner clinician profile. Removed the "growth edges" field from your profile settings. We were collecting it, but it wasn't earning its keep.
- Security patch. Updated a transitive dependency (basic-ftp) to its patched version after a recent advisory. No user data was ever exposed; this is preventive defense-in-depth.
- Behind the scenes. Bumped our deployment pipeline to the latest GitHub Actions runners and tightened the build process so library uploads can't silently fail in production.
